SPANISH PLACEMENT VERIFICATION FORM
This document is used by students interested in obtaining Spanish placement. Students may take the Spanish Placement Test at either of CWI's Assessment Services Centers. Please call to schedule an appointment two business days in advance: Aspen Creek Assessment (208) 562-3267 or Ada County Assessment (208) 562-2542. Please note: This form will be completed by an Assessment Services employee who will then submit it to One Stop Student Services for processing. Following testing, you will need to complete an Add/Drop form to register for your class. See form above in this list.

SATISFACTORY ACADEMIC PROGRESS (SAP) APPEAL CHECKLIST

Any offer of financial aid by CWI represents the college's best understanding of a student's eligibility for financial aid at the time the offer is issued. A financial aid offer is not a guarantee of eligibility or funding and is subject to conditions and/or change. This form is used to change your mailing address only. If you need to change your legal address (with changes that could affect your residency status such as moving to a different county or state), please complete a Certificate of Residency Form.

BACHELOR'S DEGREE EVALUATION REQUEST FORM
This form is used if you have earned a Bachelor’s degree from another institution and have taken courses there that will count as prerequisites for your current degree, program, or a specific course at CWI. NOTE: You must be admitted as a degree-seeking student to use this form.

CERTIFICATE OF RESIDENCY/AFFIDAVIT
Idaho code 33-2110 requires that the county you resided in for at least 12 consecutive months prior to being admitted pay part of your tuition (except for Jerome, Twin Falls, Kootenai, Ada and Canyon counties). It is your responsibility to prove legal residency by completing the form below. Some counties may require additional information or have you complete additional forms. If the county cannot certify your legal residency, you are responsible for paying the county portion of your tuition.

DUAL CREDIT REGISTRATION FORM
This form is used for current high school students registering for courses offered through College of Western Idaho's Dual Credit program. This form must be submitted for each semester the student wishes to take courses for credit in the Dual Credit program at CWI. Once this form is processed, the student will be notified and given login information to access the website. In addition to this Registration Form, a Certificate of Residency Form and a current High School Transcript may be required prior to enrollment.

MICRON COLLEGE STUDENT BACKGROUND CHECK AUTHORIZATION
This form is used by non-Micron employees who wish to take general education CWI courses offered at the Micron site. By completing this form, you authorize Micron to conduct a background check. You also agree to comply with the dress code and conduct requirements specified on the form while on the Micron site. After the form is submitted to and approved by Micron, you will be eligible to register for CWI courses taught at Micron. THIS FORM IS NOT A CWI FORM AND MUST BE FAXED TO THE MICRON REPRESENTATIVE LISTED ON THE FORM.

SELF-ADVISING REQUEST FORM
This form is used to request self-advised status. If your request is approved, you will not be required to meet with a faculty advisor prior to registration each semester. It is still recommended that you meet with an advisor the semester prior to graduation to confirm your eligibility to graduate.

Student Employment
PRE-EMPLOYMENT DOCUMENTATION
In order to begin working as a CWI Student Employee, you must document that you are eligible to work in the United States and complete a tax witholding. You must submit your I-9 and W-4 forms along with your work-study Referral Form (signed by your work-study Supervisor) before beginning employment.

WORK-STUDY TIMESHEET
Student employees should record their time hourly on a work-study Timesheet only. REMEMBER: Student employees are only allowed to work a maximum of 19 hours per week (up to your total work-study award) and may not work during your regularly scheduled class times.
